AGONISTAS DUAIS DE GLP-1/GIP NO TRATAMENTO DA OBESIDADE E DAS DOENÇAS METABÓLICAS

A obesidade é uma doença crônica e multifatorial associada à resistência à insulina, diabetes mellitus tipo 2, hipertensão arterial, doença cardiovascular, doença hepática esteatótica associada à disfunção metabólica e outras complicações sistêmicas. Nesse contexto, os agonistas duais dos receptores do polipeptídeo insulinotrópico dependente de glicose (GIP) e do peptídeo semelhante ao glucagon 1 (GLP-1), representados principalmente pela tirzepatida, ampliaram as possibilidades terapêuticas ao atuar simultaneamente sobre diferentes mecanismos relacionados à homeostase glicêmica, à ingestão alimentar e ao peso corporal. O objetivo desta revisão bibliográfica foi analisar as evidências disponíveis sobre a eficácia, a segurança e as potenciais aplicações metabólicas dos agonistas duais GLP-1/GIP no tratamento da obesidade e de doenças relacionadas. Foi realizada uma revisão narrativa da literatura, com busca prioritária na base PubMed, utilizando os descritores “tirzepatide”, “GIP”, “GLP-1”, “obesity”, “type 2 diabetes”, “metabolic dysfunction-associated steatohepatitis”, “heart failure” e “obstructive sleep apnea”, combinados por operadores booleanos. Foram priorizados ensaios clínicos randomizados, análises secundárias e metanálises publicados entre 2018 e 2026, selecionando-se estudos diretamente relacionados ao tema. As evidências demonstram reduções expressivas do peso corporal e da hemoglobina glicada, acompanhadas de efeitos favoráveis sobre pressão arterial, perfil lipídico, albuminúria e diferentes condições associadas à obesidade. Estudos recentes também apontam benefícios em esteato-hepatite associada à disfunção metabólica, apneia obstrutiva do sono e insuficiência cardíaca com fração de ejeção preservada em indivíduos com obesidade. Os eventos adversos mais frequentes são gastrointestinais e, em geral, apresentam relação com a dose e com a fase de escalonamento terapêutico. Conclui-se que os agonistas duais GLP-1/GIP constituem uma estratégia terapêutica relevante para o manejo integrado da obesidade e de suas complicações metabólicas, embora ainda sejam necessárias evidências de maior duração para estabelecer de forma mais ampla seus efeitos sobre desfechos cardiovasculares, renais e mortalidade em diferentes populações.
