Resumo

A microbiota intestinal corresponde ao conjunto de microrganismos presentes no trato gastrointestinal humano e exerce funções essenciais para a manutenção da homeostase, atuando na digestão, proteção contra patógenos e modulação da resposta imunológica. Alterações nesse ecossistema, denominadas disbiose intestinal, têm sido associadas ao desenvolvimento de doenças inflamatórias, metabólicas, respiratórias, gastrointestinais e neurocomportamentais. O objetivo deste estudo foi analisar, por meio de uma revisão sistemática da literatura, os principais padrões da microbiota intestinal e suas intercorrências sistêmicas em diferentes faixas etárias. A busca foi realizada nas bases PubMed/MEDLINE, Biblioteca Virtual em Saúde (BVS/LILACS), SciELO, Elsevier/ScienceDirect e Cochrane Library, incluindo estudos publicados entre 2020 e 2025, nos idiomas português, inglês e espanhol. A seleção dos artigos seguiu as recomendações do protocolo PRISMA 2020 e sendo registrada no PROSPERO 104571, sendo conduzida com auxílio do software Rayyan. Foram incluídos estudos observacionais, ensaios clínicos e revisões que abordassem diretamente a relação entre microbiota intestinal, disbiose e manifestações clínicas sistêmicas. Ao final do processo, 25 estudos compuseram a amostra desta revisão. Os resultados demonstraram que alterações na composição da microbiota intestinal estão associadas ao aumento da susceptibilidade a doenças alérgicas, respiratórias, metabólicas e neurocomportamentais, especialmente pela redução de bactérias produtoras de ácidos graxos de cadeia curta e pelo aumento de microrganismos pró-inflamatórios. Observou-se ainda importante participação dos eixos intestino-pulmão e intestino-cérebro na regulação imunológica e neurológica. Em relação ao uso de probióticos, os achados evidenciaram resultados variáveis, dependentes da cepa bacteriana utilizada e das características individuais da microbiota de cada paciente. Conclui-se que a microbiota intestinal exerce papel fundamental na manutenção da saúde sistêmica, sendo a preservação do equilíbrio microbiano um importante fator para prevenção de doenças e promoção da saúde ao longo da vida.
