ALTERAÇÕES INDUZIDAS PELA RADIOTERAPIA NA DENTINA E SEUS EFEITOS SOBRE OS PROCEDIMENTOS ADESIVOS RESTAURADORES

A radioterapia de cabeça e pescoço pode modificar o ambiente bucal e as propriedades da dentina, com repercussões sobre os procedimentos restauradores adesivos. Esta revisão narrativa analisa a relação entre alterações dentinárias induzidas pela radiação, resistência de união e estratégias de adesão. A síntese foi elaborada a partir de seleção temática de artigos, com consulta a registros do PubMed e páginas dos periódicos, distinguindo estudos em dentina irradiada de evidências indiretas. Os trabalhos descrevem alterações estruturais, mecânicas e enzimáticas, embora a direção das mudanças na microdureza e na composição varie entre protocolos. Diversos experimentos identificaram comprometimento da resistência de união, mas o desempenho dependeu do material, do substrato, do regime de irradiação, do envelhecimento e do método de ensaio. Os resultados sobre o momento restaurador também foram divergentes. A aplicação adicional de adesivo e a ativação ultrassônica apresentaram benefícios laboratoriais em condições específicas, sem estabelecer protocolos clínicos universais. Conclui-se que a dentina irradiada exige interpretação individualizada das evidências e planejamento restaurador integrado ao cuidado oncológico. A literatura consultada não permite definir um sistema adesivo ou um momento de restauração superior em todas as situações, nem estimar a longevidade clínica exclusivamente a partir de ensaios de resistência de união.
