Manejo da Gravidez Ectópica com Metotrexato

Resumo

Introdução: A gravidez ectópica constitui condição obstétrica potencialmente fatal, na qual o blastocisto se implanta fora da cavidade uterina, sendo a localização tubária a mais frequente. O metotrexato configura-se como a principal alternativa farmacológica ao tratamento cirúrgico em pacientes clinicamente estáveis e selecionadas. Apesar de sua ampla utilização, ainda não há uniformidade quanto aos protocolos de administração e aos fatores preditivos de resposta terapêutica. Objetivo: Descrever o papel no tratamento da gravidez ectópica, abordando suas indicações, esquemas terapêuticos, eficácia, limitações e fatores preditivos do tratamento. Metodologia: Trata-se de revisão integrativa da literatura, estruturada a partir da estratégia PICo, com buscas realizadas nas bases PubMed/MEDLINE e Embase mediante os descritores "Ectopic Pregnancy" e "Methotrexate", combinados pelo operador booleano AND. Foram incluídos artigos publicados em inglês, na íntegra, entre 2021 e 2026, totalizando 18 estudos selecionados. Resultados e Discussão: O protocolo de dose única de metotrexato mostrou-se eficaz em 70% a 95% dos casos de gravidez ectópica tubária, com desempenho comparável aos esquemas de dose dupla e múltipla, porém com menor incidência de efeitos adversos. O nível sérico inicial de hCG e sua cinética pré-tratamento destacaram-se como os principais fatores preditivos de sucesso terapêutico, ao passo que variáveis como idade materna e antecedente de gravidez ectópica não se associaram ao desfecho. Em sítios ectópicos não tubários, o metotrexato apresentou eficácia inferior à de outras modalidades terapêuticas. Identificaram-se ainda limitações relacionadas a efeitos adversos graves, ainda que infrequentes, e à aplicabilidade dos critérios de hCG em gestações por reprodução assistida. Conclusão: O metotrexato permanece como opção terapêutica eficaz e segura para a gravidez ectópica tubária em pacientes selecionadas, sendo o nível sérico de hCG o principal determinante de sua eficácia, ao passo que sua indicação em sítios não tubários e em populações específicas exige cautela e avaliação individualizada.
