Efeitos cardiovasculares e mecanismos moleculares da terapia combinada com inibidores de SGLT2 e agonistas do receptor de GLP-1: uma revisão sistemática pré-clínica

Resumo

Introdução: Inibidores do cotransportador de sódio-glicose 2 (iSGLT2) e agonistas do receptor do peptídeo semelhante ao glucagon tipo 1 (GLP-1RA) apresentam efeitos metabólicos, cardiovasculares e renais potencialmente complementares. Contudo, os mecanismos moleculares associados à combinação dessas classes ainda não estão completamente sistematizados. Objetivo: Sintetizar evidências pré-clínicas sobre os mecanismos moleculares e fisiopatológicos da combinação entre iSGLT2 e GLP-1RA, com ênfase nos efeitos cardiovasculares e renais e na possível ocorrência de efeitos aditivos ou sinérgicos. Métodos: Foi realizada revisão sistemática da literatura, com síntese qualitativa e narrativa, conforme o PRISMA 2020. Foram consultadas PubMed/MEDLINE, SciELO e Google Scholar, desde a indexação inicial até julho de 2026, sem restrição quanto ao ano de publicação. Foram incluídos estudos experimentais originais, in vitro ou in vivo, que avaliaram a combinação de pelo menos um iSGLT2 e um GLP-1RA, com desfechos moleculares, celulares, histológicos ou fisiopatológicos cardiovasculares ou renais. A seleção foi realizada por dois revisores independentes, com auxílio do Rayyan. O risco de viés foi avaliado pela ferramenta SYRCLE, para estudos animais, e por critérios adaptados do protocolo OHAT/NTP, para estudos in vitro. Resultados: Foram identificados 342 registros. Após a remoção de 68 duplicatas, 274 foram triados, 31 avaliados em texto completo e 9 incluídos. Os estudos sugeriram efeitos complementares sobre metabolismo energético, estresse oxidativo, função endotelial, inflamação e morte celular, envolvendo as vias AMPK, PKA-eNOS, NRF2/HO-1, NF-κB/NLRP3/caspase-1 e caspase-3. Oito estudos avaliaram desfechos cardiovasculares ou endoteliais e apenas um investigou desfechos renais. Conclusão: A combinação apresentou potencial efeito complementar, principalmente cardiovascular. Entretanto, a heterogeneidade dos estudos e a escassez de dados renais limitam a confirmação de sinergismo supra-aditivo.
