CIRURGIA FETAL PARA MIELOMENINGOCELE: REVOLUÇÃO TERAPÊUTICA NA MEDICINA MATERNO-FETAL

Resumo

A mielomeningocele representa a forma mais grave e funcionalmente incapacitante da espinha bífida aberta, associando exposição neural intrauterina, malformação de Chiari II, hidrocefalia, disfunção motora, bexiga neurogênica e elevado impacto familiar e socioassistencial. A cirurgia fetal modificou esse paradigma ao transformar uma anomalia antes tratada exclusivamente no período neonatal em uma condição passível de intervenção intrauterina seletiva, com potencial de reduzir lesão neural progressiva e melhorar desfechos neurológicos. O objetivo desta revisão narrativa foi analisar criticamente os fundamentos embriológicos, critérios de elegibilidade, técnicas cirúrgicas, benefícios, riscos materno-fetais e lacunas contemporâneas do reparo fetal da mielomeningocele. Foram priorizados ensaios clínicos, coortes, revisões sistemáticas, meta-análises, diretrizes e documentos de sociedades científicas publicados entre 2011 e 2026, com ênfase no estudo MOMS e em recomendações de ACOG/SMFM, NICE e Congress of Neurological Surgeons. A literatura demonstra que o reparo pré-natal reduz a necessidade de derivação liquórica, diminui a persistência da herniação do rombencéfalo e melhora a função motora em pacientes criteriosamente selecionados; contudo, associa-se a prematuridade, ruptura prematura de membranas, cicatriz uterina complexa e necessidade de seguimento materno em gestações futuras. Conclui-se que a cirurgia fetal para mielomeningocele constitui uma das maiores revoluções da medicina materno-fetal contemporânea, desde que integrada a centros de alta complexidade, aconselhamento não diretivo, vigilância longitudinal e avaliação equilibrada entre benefício fetal e risco materno.
