IMPACTO DA CIRURGIA ORTOGNÁTICA NA QUALIDADE DE VIDA DE PACIENTES COM APNEIA OBSTRUTIVA DO SONO: REVISÃO INTEGRATIVA

Resumo

Objetivo: analisar, por meio de uma revisão da literatura o impacto que a cirurgia de avanço maxilomandibular exerce na qualidade de vida relacionada à saúde de indivíduos diagnosticados com apneia obstrutiva do sono. Materiais e Métodos Trata-se de uma revisão integrativa. A pergunta da pesquisa focou em pacientes com a patologia, submetidos à cirurgia ortognática, e os reflexos em sua qualidade de vida. A busca foi realizada nas bases de dados: PubMed, Scopus e Embase. Foram incluídos artigos originais nos idiomas português, inglês e espanhol que avaliassem a qualidade de vida por meio de instrumentos e escalas validadas. A seleção foi feita por dois revisores de forma independente, utilizando ferramentas digitais para organização e remoção de estudos duplicados. Resultados Após a triagem inicial de 407 artigos, 12 estudos foram selecionados para a análise final. Os resultados indicaram que a cirurgia de avanço maxilomandibular contribui significativamente para a melhora da qualidade de vida, especialmente em pacientes com quadros de apneia moderada a grave. Observou-se uma redução consistente na gravidade da doença, evidenciada pela diminuição do Índice de Apneia e Hipopneia, já que o procedimento amplia as vias aéreas e reduz o colapso faríngeo. Além disso, houve uma redução marcante da sonolência diurna excessiva, o que melhora a produtividade e o bem-estar cotidiano dos pacientes. Instrumentos de avaliação mostraram ganhos em dimensões de saúde física, mental, vitalidade e funcionamento social. Os pacientes também relataram altos níveis de satisfação com o tratamento, inclusive nos aspectos estéticos faciais. A cirurgia foi apresentada como uma alternativa eficaz para aqueles que não conseguem aderir ao uso de dispositivos. Conclusão A revisão concluiu que o avanço maxilomandibular está associado a melhorias expressivas na qualidade de vida de pacientes com apneia obstrutiva do sono, reduzindo eventos respiratórios e impactos funcionais. Entretanto, devido à diversidade de métodos e escalas utilizadas nos estudos existentes, os autores ressaltam a necessidade de novas pesquisas com maior padronização e acompanhamento de longo prazo para fortalecer as evidências sobre os benefícios percebidos pelos pacientes.
