Celulite Infecciosa de Origem Endodôntica: Uma Revisão Integrativa sobre Etiologia, Diagnóstico e Manejo Clínico

Resumo

INTRODUÇÃO: A celulite infecciosa endodôntica é uma infecção bacteriana aguda e difusa dos tecidos moles cervicofaciais, originada de canais radiculares necróticos. Representa um problema de saúde pública pelo risco de complicações graves e potencialmente fatais, como a angina de Ludwig, e pela frequência de manejos inadequados, incluindo a prescrição incorreta de antibióticos. OBJETIVOS: Sistematizar e interpretar criticamente a literatura sobre a celulite infecciosa endodôntica, a fim de identificar seus agentes etiológicos, analisar métodos diagnósticos, avaliar estratégias de manejo e explorar as complicações associadas. METODOLOGIA: Realizou-se uma revisão integrativa com busca sistemática na base de dados PubMed, utilizando os descritores “endodontic infectious cellulitis”, “endodontic-related cellulitis” e “cellulitis of endodontic origin”. Após aplicação de critérios de inclusão e exclusão, 16 estudos foram selecionados para análise crítica. CONCLUSÃO: Os resultados confirmam a etiologia polimicrobiana, mas ampliam o paradigma ao incluir a participação de vírus e fungos em contextos específicos. O exame clínico criterioso mostrou-se superior a métodos de imagem, como a ultrassonografia, para o diagnóstico diferencial. O manejo eficaz depende primariamente da eliminação da fonte infecciosa por meio de tratamento endodôntico de qualidade, sendo a antibioticoterapia indicada apenas para casos com disseminação sistêmica, o que contraria a prática da prescrição empírica. Conclui-se que o avanço no manejo da condição exige uma mudança de paradigma, valorizando o rigor técnico e o raciocínio clínico em detrimento da dependência cultural dos antibióticos.
