Estimulação cerebral profunda no Tratamento do tremor essencial: evidências atuais

Resumo

Introdução: O tremor essencial (TE) é uma doença neurológica crônica com o potencial de impactar a qualidade de vida daqueles acometidos por ela, sobretudo em casos refratários. Nesse sentido, o presente artigo visa avaliar as evidências atuais e as perspectivas da estimulação cerebral profunda (ECP) como tratamento do TE. Metodologia: a revisão integrativa foi baseada em 35 estudos sobre a ECP no tratamento do TE. Utilizou-se a estratégia PICO e as bases de dados MEDLINE/PubMed, BVS e Science Direct para seleção dos artigos. Resultados: A ECP mostrou-se eficaz na redução do tremor essencial com melhora de até 87% no tremor contralateral e redução significativa de 66% nos marcos de pontuação da escala de classificação de tremor após 6 meses de tratamento contínuo. A segurança da ECP foi classificada como de risco aceitável, com efeitos adversos leves ou ajustáveis por meio de reprogramação. Conclusão: A ECP é uma abordagem eficaz e segura para o tratamento do TE refratário e se mostrou superior frente outras alternativas terapêuticas. Dentre os avanços tecnológicos, a ECPa é notória, mas a neuroplasticidade induzida pela estimulação crônica e a Habituação levam a perda gradual da eficácia a longo prazo.
