Dor crônica e biomarcadores inflamatórios em mulheres com obesidade: Impacto dos Fenótipos Adiposos e Lipedema

Introdução: Dor crônica em mulheres com obesidade pode resultar de interações entre distribuição de gordura (androide vs ginoide), inflamação sistêmica e fenótipos de tecido adiposo doloroso, incluindo lipedema. Objetivo: Sintetizar criticamente evidências (2020–2025) sobre a relação entre intensidade da dor e marcadores inflamatórios em mulheres com obesidade, contrastando fenótipos androide/ginoide e integrando lipedema como confundidor ou fenótipo específico. Métodos: Revisão narrativa com busca estruturada em PubMed/MEDLINE e Google Scholar (2020–2025), combinando descritores para obesidade, distribuição de gordura, dor crônica, inflamação e lipedema; seleção por relevância clínica/metodológica e extração de dados sobre fenotipagem corporal, desfechos de dor, painéis inflamatórios e controle de confundidores. Resultados: Os achados são heterogêneos; diferenças na mensuração de adiposidade (visceral vs subcutânea), nos painéis de biomarcadores e no controle de confundidores limitam comparações. Lipedema pode concentrar dor no fenótipo periférico e enviesar análises quando não identificado. Conclusão: A interpretação de dor–inflamação em mulheres com obesidade requer fenotipagem objetiva da distribuição corporal e triagem sistemática de lipedema para separar mecanismos metabólico-inflamatórios de mecanismos subcutâneos dolorosos.
