Introdução: A esquizofrenia é um transtorno psiquiátrico crônico e debilitante, com sintomas positivos, negativos e déficits cognitivos decorrentes de fatores genéticos, neurobiológicos e ambientais. Embora os antipsicóticos sejam fundamentais para controlar sintomas positivos, eles têm eficácia limitada sobre sintomas negativos e cognição, além de efeitos colaterais que afetam a adesão. Por isso, intervenções psicossociais ganham destaque, especialmente a musicoterapia, por ser uma abordagem não farmacológica, acessível, de baixo custo e bem aceita pelos pacientes. Objetivo: Identificar, por meio de revisão integrativa exploratória, a eficácia da musicoterapia como intervenção complementar no tratamento da esquizofrenia. Metodologia: A questão norteadora foi definida pela estratégia PICO (“Como a musicoterapia influencia no tratamento de pacientes com esquizofrenia?”). As buscas ocorreram entre agosto e setembro de 2025 nas bases PubMed, SciELO, EBSCOhost, Google Scholar e em ferramentas de IA, resultando em 24 estudos selecionados entre 32 publicações de 2020 a 2025. Como produto técnico, foi elaborado e validado por três especialistas um Guia Educativo ilustrado sobre musicoterapia e esquizofrenia, destinado a reduzir estigmas e ampliar o conhecimento de profissionais e do público sobre essa abordagem complementar. Discussão: A musicoterapia ativa, receptiva ou mista melhora atenção, memória de trabalho, funções executivas, linguagem e organização mental, estimulando múltiplas redes neurais e a neuroplasticidade. Também reduz ansiedade, depressão e afeto negativo, favorece a expressão emocional e aumenta autoestima, bem-estar e reconhecimento de emoções. Funcionalmente, eleva o desempenho social e ocupacional, com estudos apontando até 36% de melhora, contra 6% no controle. Seus efeitos são maiores com mais de 16 sessões e com a adaptação cultural das músicas utilizadas. Conclusão: Conclui-se que a musicoterapia é uma intervenção complementar segura e eficaz, que atua onde a medicação não basta, favorecendo a desmedicalização e a reinserção psicossocial. Sua inclusão no cuidado multidisciplinar é recomendada, embora ainda sejam necessários estudos maiores e de longo prazo para definir melhor a dose-resposta e padronizar protocolos.
