Introducción: La intervención artroscópica se asocia con dolor postoperatorio significativo en la reparación del manguito rotador., y el bloqueo interescalénico guiado por ecografía (ISB) se considera estándar para mitigar este problema, aunque presenta riesgos respiratorios. Objetivo: Explorar la efectividad del ISB en la reparación del manguito rotador. Metodología: Se realizó una revisión sistemática cualitativa mediante el modelo PRISMA, evaluando estudios publicados entre 2020 y 2024. en bases de datos Pubmed; Scopus, Cochrane. Resultados: Los resultados sugieren que el ISB ofrece analgesia eficaz y reduce el uso de opioides inicialmente, pero puede causar parálisis frénica. Comparativamente, técnicas como el bloqueo del nervio supraescapular continúan siendo seguras y efectivas sin los mismos efectos adversos. Conclusión: El ISB es efectivo para la analgesia postoperatoria, pero conlleva riesgos respiratorios significativos en pacientes vulnerables. Alternativas como bloqueos combinados pueden ofrecer seguridad y efectos analgésicos comparables, influyendo positivamente en la recuperación y satisfacción del paciente.
