O Lúpus eritematoso sistêmico (LES) é uma enfermidade autoimune crônica que acomete inúmeros órgãos e sistemas do corpo humano, marcada por períodos de atividade e calmaria e com necessidade de seguimento especializado multiprofissional. As manifestações pleuropulmonares são bastante comuns em pacientes lúpicos, entretanto o derrame pleural quiloso ou quilotórax é infrequente, ocorrendo em baixa incidência. Este estudo, por meio de uma revisão integrativa, procurou reunir evidencias acumuladas na literatura médica do comprometimento pleural quiloso como manifestação do LES que deve sempre ser pesquisada diante de casos de pacientes lúpicos com derrame pleural. O processo de acumulo de quilo na cavidade pleural pode ocorrer uni ou bilateralmente, envolvendo desde a obstrução do ducto torácico por fenômenos compressivos mediastinais, aumento da pressão hidrostática na rede linfática intratorácica, hiperpermeabilidade linfática ou fluxo transdiafragmático de ascite quilosa. Outros mecanismos podem estar envolvidos. Portanto, a pesquisa do quilotórax sempre deve ser investigada em pacientes lúpicos, independente de aspecto leitoso do líquido pleural quando puncionado ou drenado e novos estudos para melhor entendimento devem ser considerados para estabelecimento de protocolos padronizados no atendimento de pacientes lúpicos com derrame pleural do tipo quiloso.
