To achieve the proposed objectives, an interactive literature review was carried out, incorporating both clinical data and practical experiences of health professionals. The research involved searching academic databases, reviewing clinical studies and interviews with health professionals, allowing a holistic and updated approach to the topic. The importance of multidisciplinary action as an effective approach to the treatment of chronic pathologies. The integration of professionals from different areas, such as doctors, nurses, physiotherapists, psychologists and nutritionists, promotes a more complete and patient-centered approach. This results in better clinical outcomes, treatment adherence and quality of life for individuals with chronic conditions. Furthermore, this study highlights the need for continued collaboration and communication between healthcare professionals to optimize the long-term care of these patients. In summary, multidisciplinary action is essential to face the growing challenge of chronic pathologies and improve the health of the population.
