PATIENT WITH MORBID OBESITY DIAGNOSED WITH ACCIDENTAL CLOSTRIDIUM TETANI INFECTION AND RHABDOMYOLYSIS AS A COMPLICATION: A CASE STUDY

Abstract

Objective: To describe a case report of a morbidly obese patient diagnosed with accidental infection by Clostidium tetani and rhabdomyolysis as a complicating factor. Methodology: Retrospective, descriptive study, case report type, where the necessary information was taken directly from the patient's electronic medical record. Result: Patient 28 years old, with morbid obesity (130 kg), reporting fever, headache, arthralgia, myalgia and generalized rash. He has a diagnosis of severe tetanus, with rapid onset of symptoms and cardio-respiratory arrest-PCR reversed at the hospital of origin. The team reports difficulties in handling and repositioning in bed due to morbid obesity. Due to worsening respiratory patterns, he was transferred to the ICU. Patient with decreased saturation for more than 24 hours. The patient again presented frequent thoracic-abdominal spasms, hyperthermia of 38.9°C, tending to hypertension, acyanotic, anicteric, edematous. He developed rhabdomyolysis as an aggravating factor of the tetanus contractures. The neuromuscular blockade was adjusted. He developed right pleural effusion. He presented cardiorespiratory arrest without response to resuscitation maneuvers. Death was confirmed after 6 days and 16 hours of hospitalization in the intensive care unit. Conclusion: It is appropriate to bring to the academic public, and even to the healthcare public, the report of this case, rich in information that will serve as scientific support, given the scarcity of information available in the literature and the clinical importance of this disease, to guide, even further research.
