Abstract

Myocardial revascularization (CABG) in elderly patients is a crucial surgical intervention for the treatment of severe coronary artery disease, but it presents unique challenges due to the fragility and comorbidities often found in this population. This study aimed to review the existing literature to evaluate the risk factors and long-term outcomes in elderly patients undergoing CABG. The methodology involved an integrative review of articles published between 2013 and 2023, using databases such as PubMed, SciELO, LILACS, and Google Scholar. The included studies focused on CABG in patients aged 65 and older, emphasizing factors such as frailty, comorbidities, and surgical techniques. The results indicate that frailty and comorbidities, such as diabetes and heart failure, significantly increase the risk of perioperative complications and mortality. The choice of surgical technique, between conventional on-pump CABG and off-pump CABG, also plays a crucial role in outcomes, with off-pump techniques associated with a lower incidence of complications in some cases, although their long-term efficacy remains debated. Postoperative recovery and quality of life are influenced by the presence of frailty and adherence to cardiac rehabilitation programs, which have proven essential in improving long-term outcomes. In conclusion, despite the challenges, CABG can offer substantial benefits for elderly patients, provided they are carefully selected and managed with a personalized and multidisciplinary approach. Continuous research and the development of tailored rehabilitation strategies are essential to optimize outcomes and improve the quality of life in this growing population.
