Abstract

Non-communicable chronic diseases (NCDs), such as diabetes, hypertension, and heart diseases, continue to pose significant public health challenges worldwide. This article reviews innovative approaches to treating these conditions, focusing on personalized treatment, care integration, and digital technologies to enhance disease management and treatment adherence. The introduction contextualizes the importance of these strategies, highlighting the benefits of addressing NCDs through patient-centered models. The methodology involves a systematic analysis of published studies, identifying best practices and emerging strategies. The results indicate that personalized medicine is key to tailoring treatments to individual patient needs, significantly improving clinical outcomes. Integrated care across specialties provides a holistic management model, as studies demonstrate improvements in treatment access and patients' quality of life. Telehealth and mobile applications facilitate self-management and promote health education, particularly among remote populations or those with limited access to healthcare services. The discussion emphasizes the implications of these findings for clinical practice and health policy, underscoring the need for more flexible and integrated models that consider local characteristics and patient preferences. Evidence-based practices and innovative approaches like telehealth are crucial to overcoming the challenges of NCDs, which require multidisciplinary and sustainable strategies. In conclusion, personalization, care integration, and digital technologies are promising approaches for effectively managing NCDs. These strategies have the potential to significantly improve quality of life and health outcomes, providing more efficient treatment models adapted to modern challenges.
