Introdução: A nefropatia induzida por contraste (NIC) é uma complicação renal que surge após a administração de agentes de contraste, particularmente em pacientes com diabetes e insuficiência renal. Muitas vezes subdiagnosticada, a NIC pode levar à progressão da doença renal crônica e ao aumento da mortalidade, afetando até 20% dos pacientes em risco. Estratégias de prevenção, como hidratação adequada e escolha apropriada de agentes, são fundamentais. Objetivo: Realizar uma revisão sistemática da literatura sobre NIC, focando em fatores de risco, mecanismos fisiopatológicos, estratégias de prevenção e opções de tratamento, visando otimizar práticas clínicas e promover a segurança do paciente. Metodologia: A revisão foi conduzida nas bases de dados PubMed e BVS, utilizando critérios rigorosos de inclusão e exclusão, de acordo com as categorias: mecanismos patológicos, fatores de risco, estratégias de prevenção, tratamento e desfechos clínicos. Resultados e Discussão: Os mecanismos da NIC incluem toxicidade dos agentes de contraste, hipoperfusão renal, estresse oxidativo e inflamação, que agravam a lesão renal, especialmente em pacientes com condições pré-existentes. Os fatores de risco são intrínsecos, como doenças renais e diabetes, e extrínsecos, como tipos e doses de contraste. A prevenção e o tratamento dependem da hidratação, identificação precoce e monitoramento contínuo da função renal. A educação médica é crucial para a implementação de diretrizes eficazes. Conclusão: Compreender os mecanismos e fatores de risco da NIC é fundamental para implementar estratégias preventivas e terapêuticas. A revisão enfatiza a necessidade de uma abordagem multidisciplinar para melhorar os resultados clínicos em pacientes expostos a agentes de contraste.
