Resumo

Objetivos:  O objetivo deste estudo foi avaliar uma abordagem não-cirúrgica da peri-implantite por meio de desbridamento mecânico e desintoxicação local, deixando o tecido de granulação nas cavidades do peri-implante.

Materiais e Métodos: Foi realizada uma análise dos resultados de 2 protocolos não cirúrgicos: desbridamento mecânico mais desintoxicação química com clorexidina a 0,2% e cloridrato de clortetraciclina (teste) e desbridamento mecânico sozinho (controle). As medidas foram coletadas no início, 3, 24 e 36 meses após o tratamento.

Resultados: Setenta e cinco implantes com peri-implantite foram incluídos. Em uma avaliação de 36 meses, a profundidade média da sondagem foi de 3,15 ± 0,32 mm para o grupo teste e 5,97 ± 0,90 mm para o grupo controle, sendo a diferença significativa. Uma melhora no nível ósseo marginal foi registrada para o grupo de teste, independentemente de o grupo controle mostrar uma perda óssea marginal adicional. O sucesso do tratamento foi de 100% no grupo teste e 31,4% no grupo controle.

Conclusão: O desbridamento não cirúrgico do implante por si só não pareceu eficiente no tratamento da peri-implantite. São necessários estudos longitudinais adicionais para avaliar a eficácia do desbridamento mecânico com desintoxicação química e remoção de tecidos de granulação contra o desbridamento mecânico somente com desintoxicação química.
