Abstract
The management of anatomically and physiologically difficult airways is a significant challenge in emergency settings, particularly in patients with devices such as the Montgomery tube. This is a qualitative case report that describes the approach to a patient admitted with severe respiratory insufficiency, hypercapnia, and respiratory acidosis in southern Bahia, Brazil. Awake orotracheal intubation was performed using videolaryngoscopy, followed by emergency bronchoscopy, which identified and treated an obstructive tracheal granuloma. The techniques employed ensured patient stabilization and highlighted the effectiveness of videolaryngoscopy and bronchoscopy as essential tools for managing complex airways. This clinical case reinforces the need for medical team training and the availability of specialized equipment in emergency units.
