Abstract

The process of tooth eruption begins in the embryonic stage, where it goes through several stages until it completely erupts into the oral cavity. During the dental chronology, the tooth can be influenced by various factors, including late eruption due to excess fibrous gingival tissue covering the crown region of the tooth. This makes ulectomy surgery necessary, which consists of removing gingival fibrosis from unerupted permanent teeth. The aim of this article is to report a clinical case of ulectomy in a pediatric patient with delayed eruption of permanent central incisors. A 7-year-old male patient came to the pediatric dentistry clinic at the Centro Universitário Brasileiro-UNIBRA, accompanied by his guardian, complaining that his anterior teeth were “not erupting”. Anamnesis revealed no systemic alterations and there were no reports of extrinsic factors that could result in this delay, such as local alveolar trauma or early loss of the deciduous tooth. On clinical examination, the presence of a thick band of gingival tissue with a paler color was noted on element 11 and, to the touch, it was evident that the crown of the tooth had not erupted. For radiographic analysis of the case, a modified periapical technique was performed, as well as a panoramic view, where it was possible to see the presence of the tooth corresponding to stage 8 of the Nolla classification, which shows the real need for surgery, since, even with ⅔ root formation, the tooth was not in the mouth. After 19 days of observation, tooth 11 was properly positioned. However, this case report was differentiated by the extraction of teeth 52 and 62, which was necessary in order to make room for tooth 21 to erupt into the oral cavity. The conclusion of this case is that the ulectomy surgical approach is effective for the treatment of retained dental elements, in order to provide satisfactory esthetic-functional characteristics.
