Abstract

O HIV em crianças continua sendo um desafio de saúde global, principalmente em regiões de baixa e média renda. Além das barreiras clínicas e epidemiológicas, as crianças vivendo com HIV enfrentam o peso adicional do estigma, que afeta profundamente a qualidade da prestação de cuidados e o bem-estar psicológico. Este artigo de revisão explora a complexa relação entre o estigma social associado ao HIV e a adesão ao tratamento antirretroviral (TARV) em crianças vivendo com o vírus. A análise evidencia que, embora o TARV tenha transformado o prognóstico de crianças soropositivas, garantindo-lhes maior longevidade e qualidade de vida, a persistência do estigma nas esferas individual, comunitária e estrutural compromete significativamente a adesão ao tratamento. O estigma, profundamente enraizado em questões sociais e culturais, afeta não apenas as crianças, mas também seus cuidadores, criando um ciclo de vergonha, exclusão e isolamento que reduz o acesso aos cuidados de saúde e prejudica a continuidade do tratamento. A adesão ao TARV deve ser vista sob uma perspectiva ampliada, que leve em consideração as barreiras psicossociais e estruturais enfrentadas por essa população. Nesse contexto, conclui-se que a efetividade do tratamento depende de uma abordagem colaborativa e multidisciplinar, que não só reconheça as especificidades do HIV em crianças, mas também enfrente ativamente as raízes do estigma, promovendo um cuidado mais justo e equitativo.
