Extensive external resorption on the buccal surface of the left upper premolar

Abstract

Patient, Caucasian, female, 25 years old, without systemic problems, was referred to the private office of an endodontic specialist for treatment of tooth 25. Cone beam computed tomography revealed extensive resorption affecting the buccal surface. In the periapical radiographic examination, the presence of external root resorption was observed in the cervical and middle third of the root canal, with undefined limits and, apparently, without communication with the adjacent bone, since the greatest involvement was on the buccal surface, according to tomography.  Initially, negotiation of the root canal space was done using a number 10 K-Flexofile file (Dentsply Maillefer, Ballaigues, Switzerland) and abundant irrigation was done using 2% Chlorhexidine solution and saline solution. The working length was determined with an apical locator, Irrot Apex (Easy) followed by irrigation with the mentioned chemicals. The foraminal patency was performed with the 40.01.  The canal was prepared using the Easy ProDesign Logic 2 system: #40.05.  The filling of the root canal system using the HB (easy) single cone technique #30.05 in the mesial canals and #40.05 associated with Bio C Sealer cement. Figure 2. To seal the resorption, flap surgery was performed to correctly fill the area of ​​buccal resorption with CIMMO PSB HP cement. It is concluded that the prognosis of external readicular resorption is dependent on computed tomography to carry out the correct therapeutic approach, endodontic treatment complemented by surgery to achieve success after 4 years of follow-up.
